Housing for Health Orange County is on a mission to empower individuals across the county experiencing homelessness by providing comprehensive support and resources, and guiding them towards sustainable housing solutions through collaborative efforts with local housing providers and innovative services. Our commitment extends beyond just housing; we aim to break the cycle of homelessness by supporting the whole human, addressing their physical and mental health needs.
WHAT WE DO
OUR STORY
Housing for Health Orange County (HHOC) was founded to empower unhoused individuals by addressing not only their housing needs but also their physical and mental health through partnerships with local service providers and comprehensive supportive services.
As a regional entity, HHOC exists to dismantle barriers within regulatory bodies, uniting resources across cities to tackle homelessness as a shared community issue. The organization initially began by offering Housing Navigation Services; designed to support individuals at risk of losing housing or those experiencing homelessness; Housing Deposits, providing up to $5,000 to assist with rental and move-in costs; and Housing Sustainability and Tenancy Services, which focus on long-term housing stability through financial management, community integration, and health management support.
HHOC has since expanded its offerings to include Day Habilitation Services, providing life skills development; Enhanced Care Management, addressing complex health needs; and, most recently, Short-term Post-Hospitalization housing support.
HHOC’s story is one of collaboration and scalable impact. The organization was built to bridge gaps in funding, resources, and services for Orange County’s most vulnerable populations. Today, HHOC is contracted with CalOptima, which serves as the Managed Care Provider for Orange County, extending HHOC’s reach through CalAIM to support a diverse client base.
Looking ahead, we at HHOC are committed to continued growth by continuing to support local providers with a variety of essential functions, so they can focus more on direct, personal care. Our approach has already made a meaningful impact in Orange County, attracting attention from state and regional organizations as a promising model for addressing homelessness. With this momentum, we hope to expand our reach and bring effective solutions to communities beyond our county borders.
